I got this one. Don't remember exactly but bought it at Rs 2500. Its an Unlocked 3G Wi-Fi Dongle. So, I am using Reliance 3G, 15GB Plan at Rs 1200.

I purchased it from Flipkart.

No no, mine is an unlocked version. Any operator can be used with this dongle.

Yes, dongle look almost same but I think PTCL is having a customised version of 9 Mbps. And this 21 Mbps is universal and available for all.
