My connection is 5 years old, bro. But I upgraded to HD connection last year in January 2015. Now the d2h guy says he is not going to pay anything for STB and will take back the STB.

bro - there are three choices -
1) first ask them how much they will pay u. if they r ready to pay all the amount then it is ok as u can't do anything in it. if u are really serious on it then do write a mail to trai including d2h mail id too and write all the things in that along with trai rules and regulations regarding the same.
2) u can sell ur d2h stb to anyone. which i mean if u have 3000 rs. left in ur a/c then sell it to any one. in d2h u can sell ur connection to anyone without anything being charged and the name and all those things will be transferred too and all these things will be free of cost.
3) if 1st and 2nd choices are not fulfilled or partially fulfilled then use it till it expires.
D2H local area manager is coming with cheque tonight. He says I have to return Antenna, LNB and Cable as well according to their CAF form. I don't see anything mentioned about returning of antenna, LNB and cable in their form. Is it the rule or they are taking it out of anger because I have decided to terminate my connection?

I think they are doing this out of anger as I have never seen or heard anything like this.
But I would suggest you to accept the cheque if they are returning full amount and also take acknowledgement from them of returning your devices.
Better to move on with a better service provider.

bro... u just changed the lnb and cable then y the hell u will return it back to d2h team. u paid for both the things then both the things belong to u only na.
how much they will return u ? full amount. if yes then decision is urs.
or if full amount is not there then be stick to d2h and try to sell it to some other potential customer on ur own so that u will get all the money.

My suggestion would be to move on as no profit in fighting with such arrogant people who do not want you as a customer.
